Here are the instructions to customize the form.

1. click ‘Edit’ button on the top left of this page to get in and edit the page.

2. Scroll down to see the form in the box below.

3. Click anywhere on the form and you will see a box pop up. click on the ‘pencil’ icon.

4. Another box will pop up. Under the Content tab click "‘edit form fields.”

5. Here you can add more fields. The box you want her to write in is called ‘Text’ that’s a text box. You can then make certain form fields required by moving the toggle.

6. Once you have all the forms done you can then select the ‘post-submit’ this is what will show up after someone submits the form. You can customize this.

7. for submission I have it goign to the gmail you gave me but double check I entered it correctly.

DONT’T FORGET TO SAVE BEFORE LEAVING THE PAGE. You can save it often if you like too.
